This is not a remote position - located in Fort Smith, AR

You'll engage with the patient's entire family to help them and other caregivers understand and support the process. GENERAL PURPOSE

Provides nursing assessment, planning and care telephonically to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care after regular business hours and/or on weekends. Supports the Provider Relations activities of the organization.

ESSENTIAL JOB DUTIES

* Responds to all telephonic client care issues and provides instructions for patient/family following or changing plan of care

* Initiates communication with various on call staff, attending physicians, hospice medical directors, referral sources, other hospice staff members and maintains communication as needed with the appropriate administrator on call (AOC)

* Receives client referrals, reviews hospice services and philosophy with families/patients, obtains orders, enters into EMR, verifies insurance via electronic modalities, obtains authorization to admit from AOC, schedules on call staff for assessment and admission visits

* Processes and maintains an up-to-date patient EMR

* Collects, collates and maintains specific branch information to be utilized during the work day

Must adhere to the Company's Code of Conduct and Business Ethics policy including documentation and reporting responsibilities.



Position Requirements

QUALIFICATIONS

* Registered Nurse with a current, unrestricted state license, and be eligible to obtain other state licensure

* Minimum of 5 years general nursing experience with current Hospice experience preferred. Experience in end of life care and pain management

* Obtains CHPN certification within one year of employment if not currently certified

* Ability to work weekday nights and weekends

* Must be capable of maintaining regular attendance

KNOWLEDGE, SKILLS, ABILITIES & BEHAVIORS

* Familiar with policies of the hospice and state and federal regulations pertaining to nursing services including Medicare certification

* Commitment to company philosophy of care and service standards

* Excellent assessment, nursing judgment and communication skills

* Computer skills including Microsoft platform and ability to learn and utilize company EMR software

* Ability to work with minimal supervision

* Must be able to maintain confidentiality regarding patient, employee and company proprietary information

* Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels

PHYSICAL AND SENSORY REQUIREMENTS

Light work involves lifting no more than 20 pounds at a time with frequent lifting or carrying of objects weighing up to 10 pounds. Even though the weight lifted may be very little, a job is in this category when it requires a good deal of walking or standing, or when it involves sitting most of the time with some pushing and pulling of arm or leg controls. To be considered capable of performing a full or wide range of light work, you must have the ability to do substantially all of these activities. If someone can do light work, we determine that he or she can also do sedentary work, unless there are additional limiting factors such as loss of fine dexterity or inability to sit for long periods of time.
