As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education. Qualifications: - A heart to serve patients and families and a passion for providing the best possible care - Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing - Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred - Required: Reliable transportation.
