active member of the medical community in your territory, you will
provide both ongoing palliative care and hospice service and

call to care for others. Qualifications: - A heart to serve
patients and families and a passion for providing the best possible
care - Education: Graduate of an accredited Certificate, Diploma,
Associate or Baccalaureate School of Nursing - Experience: 2 years
experience within the last 5 years in chronic disease
management/long-term care. Hospice background preferred - Required:
Reliable transportation. Ability to sit, stand, bend, move
intermittently and lift at least 80-100 lbs and bear the weight of
an average adult effectively We’ve worked hard to build a caring
