Our Care Management approach is essential for providing

forefront. As a Care Manager, you will assist patients throughout
their hospital experience, from admission to discharge. By
collaborating closely with physicians, nurses, and other healthcare
professionals, you will ensure that each patient receives timely
and appropriate care. Additionally, you will support individuals
with complex or high-risk psychosocial needs, making a significant
impact on their lives. The Care Manager, as a member of the Care
Management team, plays a vital role in facilitating the discharge
planning process in partnership with the healthcare team.
Furthermore, the Care Manager may engage with patients facing
complex psychosocial challenges as assigned. All responsibilities
should be executed in alignment with our organization’s mission,
values, and Service Standards. Position Overview: Key
Responsibilities: Initiate and oversee discharge planning from the
first day of admission. Work collaboratively with a
multidisciplinary team to ensure seamless patient transitions.
Provide compassionate and strategic support to patients with
complex needs. Enhance performance by utilizing best practices and
data-driven insights. Embody our organization’s mission, values,
and Service Standards in every interaction. Qualifications
Education: A Bachelor of Science in Nursing (BSN) is required.
Licensure: A current RN license in the state of employment is
mandatory. Experience: 2–3 years of experience in an acute care
hospital setting is preferred. Experience in Care Management or
Utilization Management is advantageous. Certifications: BLS (CPR)
certification is required upon hire or within 90 days. Case
Management certification is preferred. Skills & Knowledge:
Excellent verbal and written communication skills. Strong
assessment, time management, and critical thinking skills. Ability
to effectively engage with individuals at all levels of the
organization. Competence in providing age-specific care for
neonatal, pediatric, adolescent, adult, and geriatric patients.
Familiarity with InterQual or Milliman Care Guidelines is
