Places to Work." Overview: The Social Worker I - Population Health,
as part of the Population Health team, intervenes with patients who
have medical needs. In addition, the Social Worker I offers crisis
intervention to patients and families and coordinates and
facilitates the development of a discharge plan of care. This role
receives referrals for individuals from at-risk populations from
interdisciplinary team members (including physicians, nurses,
advanced practitioners and other members of the care team). The
Social Worker I performs duties and responsibilities in a manner
consistent with the Mercy mission, values, and Service Standards.
- Education Bachelor s Degree in social work or health related
field, or licensed social worker with related experience. Licensure
Licensure in Social Work in the state of employment Experience Two
to five years clinical experience with supervisory skills
essential. Certification/Registration BLS (CPR) at hire date,
required, or within 90 days of hire Licensure Current license in
Social Work, master s level or above, in the state of employment.
Experience Minimum 2 years experience in acute care hospital
setting, adult experience preferred Certification/Registration BLS
(CPR) at hire date, required, or within 90 days of hire Other
Knowledge, Skills & Abilities
- Good oral and written communication and facilitation
- Good assessment, time management, & critical thinking
- Good record keeping & computer skills.
- Ability to assess, care and treat patients be consistent with
their specific age-related needs.
- Skills and knowledge of human behavior and clinical aspects of
medical Social Work.
- Ability to interact effectively with a variety of people and
